A diagnosis of diabetes can change your life. but it doesn't have to diminish your life.By combining your doctor's treatment plan with some basic knowledge and a few new skills, it is possible to keep diabetes under control and minimize your risk for complications.United Regional Health Care System offers a Diabetes Self-Management Education Program which has been recognized by the American Diabetes Association.The "Self-Management" concept means that YOU are the central focus, and your "team" includes your physician, certified diabetes educator, registered dietitian and many other health care professionals.
Diabetes Overview
A series of comprehensive educational classes teach the patient and family self-management skills to reduce the risk of complications. Classes are offered regularly and family participation is encouraged. Follow-up is provided following the completion of the series of classes. Together you and your diabetes team will review your progress in managing the disease and enjoying an active healthy life. The outpatient program is based on the American Diabetes Association National Standards for Diabetes Self-management Education. Reimbursement for outpatient education is offered through Medicare and most major insurance companies.
In order to enroll in this program, a referral must be made by your physician. We will then contact you to set up an initial appointment. Patients should have a baseline A1C and lipid panel prior to the first visit.
Our mission is to provide quality comprehensive diabetes self-management education following the American Diabetes Association Standards of Care and to be accessible to all people in Wichita Falls and the surrounding area. We believe that patient and family education is essential and the right of every person.

The Diabetes Self-Management Program is located on the Eighth Street Campus of United Regional Health Care System at 1600 Eighth Street. The office is located on the first floor of the building. Enter the main entrance and follow the hallway to the right. At the end of the hallway (four elevators will be directly in front of you), turn left and proceed down to the end of the hallway.
