Emergency Department to Emergency Department Transfer Hospital to Hospital Transfer Physician Office to ED Transfer Physician Office to Hospital Transfer Physician Representative Network		Hospital to Hospital Transfer A call should be placed from the transferring hospital physician to the United Regional physician he or she wants to care for the patient. A call may also be placed to a Cogent physician at (940) 764-3986 or to the Residents at (940) 867-6829. The receiving physician will be happy to assist in the transfer and will ask the transferring physician some questions about patient to help determine level of care needed. The receiving physician will inform the House Supervisor of the acceptance of the patient. A call should be placed from transferring hospital to the House Supervisor at (940) 781-1333 to provide patient demographics, vital signs, weight, IVs, status/report to help us determine appropriate level of care for patient. The transferring hospital should fax the patient’s face sheet to bed control at United Regional at (940) 764-3368. Report will be called to the unit the patient is being admitted to.
