Hospital to Hospital Transfer

 

What is Access Management?

 

Access to care, accurate level of care and continuous monitoring against nationally recognized acute care criteria will be the primary function of the Access Nurse. An Access Nurse will be available 24 hours a day 7 days a week to receive referral calls for direct inpatient admissions and inpatient transfers. The Access Nurse may be reached at (940)764-3333.

 

Hospital to Hospital Transfer

 

A call should be placed from the transferring hospital physician to the United Regional physician he or she wants to care for the patient.

 

A call may also be placed to a United Regional Hospitalist at (940)764-3986 or to the Residents at (940) 867-6829.

 

The receiving physician will assist in the transfer and will ask the transferring physician clinical information about patient to help determine the level of care required.

 

The receiving physician will then inform the House Supervisor of the acceptance of the patient.

 

A call should be placed from transferring hospital to the Access Nurse at (940)764-3333 to provide patient demographics including:

 

• Working Diagnosis

• Where is the patient coming from

• Pertinent Clinical Information

• Vital Signs

• Pertinent History and Physical findings

• Plan for Treatment

• Admitting Doctor

• Callers name and number

• The procedure to be done (if any needed).

 

The transferring hospital should fax the patient’s face sheet to United Regional at (940)764-3368.

 

Report will be called to the unit the patient is being admitted to.
