Social Work and Care Coordination
If you or your family have special concerns, our social workers can provide information and assistance.
Services include:
- Discharge planning
- Post-hospital planning
- Arrangements for home health services or extended care facilities
- Referral to community agencies
- Counseling or financial information on Medicare, Supplemental Security Income, disability or Medicaid nursing home care
Care Coordination
A Care Coordinator, who is a registered nurse, will review your plan of care and expected outcomes of treatment that your physician has ordered. The Care Coordinator will serve as your advocate to ensure the services ordered are delivered in a timely, efficient fashion.
