Most credentialed providers are required to complete New Provider Orientation before practicing at United Regional. New Provider Orientation consists of both online and in-person orientation pieces. In person orientation is a one-on-one orientation that is specialty specific and includes your Epic training, department tours and orientation, and technology setup. All new providers are required to complete Epic and hospital online learning modules prior to arrival.

Thank you for your interest in joining the Medical Staff of United Regional Health Care.

Credentialing Checklist and Documents
Make sure all uploaded files are .jpg or image files. Do not submit Word or Excel documents as this may cause system errors.
- Copy of valid/current government issue I.D.
 - Copy of your TX State license
 - Any BLS/ACLS/ATLS/PALS/NPR certifications you possess
 - Board Certification
 - Case/Treatment Logs (Organized)
 - CME/CE completed in the past 24 months
 - Collaborative Agreement
 - Copy of a current, valid malpractice insurance certificate
 - Current health statement with documentation of recent TB test results
 - Curriculum Vitae
 - Controlled Substance License
 - DD Form 214, Discharge Papers and Separation Documents
 - DEA Registration
 - ECFMG Certificate
 - Quality passport quality photo (no copies of IDs)
 - Residency logs/procedure lists for the last 2 years (Required if GME/Training completed within past two years)
 
Tips for faster credentialing Application Processing
List fax numbers and/or email addresses for all professional references, previous/current employer(s), and hospital affiliations. Submitting valid email addresses and fax numbers will reduce application delays and help expedite processing.
Ensure you include the delineation of privileges when you submit your application. Your application will not be considered complete without this.
